Working together, the treatment team, referring
professional, family and patient develop an appropriate
discharge and continuing care plan. Our goal is to
assist the individual in maintaining ongoing recovery.
The continuing care plan may include a referral to a
continuing care group, community based support group,
recovery residence or outpatient therapy.
