Admission and Assessment Information

The best way to make a referral is to call our toll-free number (1-800-264-5640). We have staff specifically dedicated to leading the referral source and/or family through the referral process, including scheduling a free face-to-face assessment in their communities. These staff are available 24 hours a day. Calls can be made 24 hours per day, 7 days per week.

The Mobile Assessment Team has been specifically trained to understand level of care criteria and community resources that may be available to the patient and family in question, and provide limited case management services to these families. These screenings are provided under the direction of clinical and medical leaders at the hospital, with a physician available by telephone to discuss any specific screenings.
